Moderation log

Every moderation decision is recorded here. The log is append-only: the database rejects any attempt to update or delete an entry, so a decision cannot be quietly rewritten later.

This is what makes our claims checkable rather than merely stated. If we ever removed a review for a reason that was not policy, it would have to appear here — or not appear at all, which the gap would show.
